  1. The two (2) most recent completed financial audits
  2. Simple Budget for current year.
  3. A listing of members of the governing body, when they first took office, and the expected end date of their current term.
  4. The Mission Statement and By-laws of the governing body.
  5. A well-defined list of the services provided and instructions on how a member of the public applies for services, grants, or other benefits offered by the organization (as appropriate).
  6. Location and contact information of the entity, a listing of the executive and management staff, including the executive director, and their professional contact information.
